feat(security): vault wall, path guard, and utils
- utils/paths.py: pyra_home(), ensure_dir(), safe_chmod(), expand()
- security/boundaries.py: VaultAccessError, PyraSecurityError,
assert_safe_path() (called before every file read), check_vault_lock()
