The unique constraint on `(user_id, connection_id, cloud_item_id, version_key)` prevents duplicate inserts, raising `IntegrityError`. - **Fix:** When an evicted entry exists for the same version_key, reactivate it (clear `evicted_at`, update `object_key` and `size_bytes`) instead of inserting.
